If you are in an accident or injure yourself, please notify your supervisor immediately, complete the accident / incident report and attach any photos. Be sure to submit the police report number, if one is available. This helps in getting pertinent information about the accident to our insurance company.

It is imperative we get this submitted to our insurance company as soon as possible. This needs to be filled out before returning to work the next day, unless you are injured. In that case, give as much information to your supervisor so that he may submit the claim to the corporate office.
